Office Hours
Our office is open from 8:30 AM to 5 PM, Monday through Friday. If you are an existing patient of Dr. Lim (you have seen him as a patient before) and an emergency arises after office hours, please go to the nearest emergency room. Please ask your emergency room provider to notify Dr. Lim.

New Patients
If you are a new patient (you have not seen Dr. Lim as a patient before) please provide us with as many of the items listed below as you can.  If you are being referred by another doctor, please ask the referring physician to fax us any appropriate clinical information. If you are already a patient of Dr. Lim, we want to thank you for continuing your relationship with us. We value the opportunity to continue providing you with excellent urological care.

Fees
We know that you have choices when it comes to your urological care.  We want to provide you with the best service and the most convenient appointment time.  Fees vary depending on the level and the complexity of service.  If you have any questions about our fees, please call our office.  We will gladly answer any questions you may have.

If you are a new patient, we suggest that you call or check out the website of your insurance company to make sure that Dr. Lim is on your plan. Dr. Lim is a urology provider for many managed care organizations and programs (PPO, HMO, POS, and EPO). It is the patient's responsibility to know the co-pay and deductibles for his or her insurance plan. HMO patients need a referral form from the referring physician.

Co-pays or deductibles are collected before seeing Dr. Lim. We accept Visa, MasterCard, Checks, and Cash.

Dr. Lim is a provider for the following insurance plans.  If you don’t see your insurance company on this list, please verify that Dr. Lim is a provider for your plan. Insurance contracts change from year to year. Please check with our office to verify eligibility of coverage if you have any questions.

If you cannot come to your scheduled appointment, please call us at least 24 hours in advance to reschedule. Missed appointments will result in a $25 "no show" charge.
